Subject: Important Information Regarding Your Care: Dr. Shogren

Dear Patient,

I am writing to inform you that Dr. Shogren is currently experiencing health issues and is taking an immediate medical leave of absence from her practice.

At this time, we do not have an estimated date for her return to practice, as the duration of her leave will depend on her recovery. As a result, Dr. Shogren is currently unable to see patients, provide medical advice, or issue new prescriptions/refills.

Seeking Alternative Care: Your continued care and mental health are our highest priority. We have been actively attempting to secure a covering psychiatrist to provide care for Dr. Shogren’s patients during her absence. While these efforts are ongoing, we have not yet been able to secure coverage. To ensure there is no delay or lapse in your treatment, you should seek care from an alternative psychiatrist or healthcare provider as soon as possible.

    •    If you are in an immediate mental health crisis or experiencing a health emergency, please call 911 or go to the nearest emergency room.

    •    To find a new provider, you may contact your health insurance company for a list of in-network psychiatrists or reach out to other resources such as the Dallas County Medical Society for referral assistance.

Access to Your Medical Records: Your medical records remain confidential and secure. To facilitate a smooth transition of your care, you may request a copy of your records or have them transferred directly to a new provider. To request your records, please complete the "Authorization for Release of Confidential Information" form and return it via secure email at frontdesk@drshogren.com, our secure fax number (972)392-4407, or by mail at 5485 Belt Line Road, Suite 160, Dallas, Texas 77254.We will process these requests as quickly as possible.

Prescription Refills: Please note that we cannot provide bridge refills during this leave. We suggest contacting your primary care physician or your pharmacist to see if they can provide a temporary supply while you establish care with a new psychiatrist.

We understand this news may be distressing and apologize for the disruption in your care. We appreciate your understanding and wish you the best in your continued health journey. We plan to provide an email update when we have a better sense for if/when Dr. Shogren will return from her medical leave.

Sincerely,

The Office of Dawn Shogren, M.D., P.A.

End of the Public Health Emergency (PHE)

The COVID-19 Public Health Emergency (PHE) was initially enacted in January 2020.  After nearly three years and 11 extensions, the Biden administration on January 30, 2023 announced the COVID-19 Public Health Emergency would end May 11, 2023.

Telemedicine

Telemedicine cannot be used to prescribe controlled substances after May 11, 2023.

Health Insurance Portability and Accountability Act (HIPAA) potential penalty waivers will end. This allows providers to communicate with patients through telehealth on a smartphone, for example, without violating privacy laws and incurring penalties.
