Subject: Permanent Closure of Practice: Dr. Shogren
Dear Patient,
On December 22, 2025, we sent a message informing you of Dr. Shogren’s medical leave. Due to ongoing health issues, Dr. Shogren has made the difficult decision to close her practice permanently. We appreciate your understanding and wish you the best in your continued health journey.
Dr. Shogren has asked me to share that practicing psychiatry has been her life’s passion. She chose this field because she valued the opportunity to support health and change through a direct connection with her patients. This work provided her with a consistent sense of purpose, and she found the process of helping others reach their goals to be a rewarding part of her career. Although her health now requires her to close the practice, she remains grateful for the connections she shared with those in her care.
Seeking Alternative Care: Your continued care and mental health are our highest priority. We have been actively working to secure a covering physician, and while we have been unable to do so, we are providing a list of physicians that are accepting new patients. To ensure there is no delay or lapse in your treatment, you should seek care from an alternative psychiatrist or healthcare provider as soon as possible.
If you are in an immediate mental health crisis or experiencing a health emergency, please call 911 or go to the nearest emergency room.
To find a new provider, you may contact your health insurance company for a list of in-network psychiatrists or reach out to other resources such as the Dallas County Medical Society for referral assistance.
Access to Your Medical Records: Effective February 9, 2026 we have retained Cariend, a medical records custodian, to
manage the records for the next 7 years. Cariend is not a medical provider and does not handle billing, medications, or referrals for the closed practice.
Requestors should be prepared to complete the Records Release Authorization Form and provide a copy of government-issued photo ID, as required by HIPAA. The fastest way to process requests for copies of medical records is to visit Cariend’s website and follow the instructions posted here: https://www.cariend.com/request-records/
If you do not have access to a computer or internet, Cariend can be reached by phone at 404-991-7080 – please note the HIPAA laws do not allow medical records to be released based on a phone or email request.

Prescription Refills: Please note that we cannot provide refills. We suggest contacting your primary care physician or your pharmacist to see if they can provide a temporary supply while you establish care with a new psychiatrist.

End of the Public Health Emergency (PHE)
The COVID-19 Public Health Emergency (PHE) was initially enacted in January 2020. After nearly three years and 11 extensions, the Biden administration on January 30, 2023 announced the COVID-19 Public Health Emergency would end May 11, 2023.
Telemedicine
Telemedicine cannot be used to prescribe controlled substances after May 11, 2023.
Health Insurance Portability and Accountability Act (HIPAA) potential penalty waivers will end. This allows providers to communicate with patients through telehealth on a smartphone, for example, without violating privacy laws and incurring penalties.
Mental health services will necessitate an in-person visit within six months of initial assessment and every 12 months thereafter;
Medicare will no longer cover audio-only visits for physical health encounters.
